We will automatically forward the payment to your facility and ensure your payment is reconciled. Please allow up to two weeks for processing. You may receive additional statements during the processing period.

Your statement was generated using the information provided by your treatment provider. If you believe any of the information included in the statement is incorrect, please contact your treatment provider directly. We are unable to assist with correcting this information.

  • The name, address and phone number of your rendering provider
  • The services provided by your rendering provider
  • The amounts paid by your insurance company for these services
  • The amounts remaining which you owe, these are typically composed of your deductible, co-pay, or co-insurance

Why Did I Receive A Bill?

Whenever you utilize your insurance you will almost always owe a co-pay or co-insurance with your treatment. This is sometimes a fixed amount, such as $30 per appointment, and sometimes it is a percentage of the total amount due. You must pay this co-pay or co-insurance in order for your insurance company to pay their share.


You will be responsible for some amount for all treatment until you reach your out of pocket max. An out of pocket max is the maximum amount you will be required to pay in a given year for treatment. When undergoing treatment it is typical for patients to hit their out of pocket max fairly quickly. Your statement total likely represents your total out of pocket max for the year.

Why Did I Get A Statement?

Your insurance company has indicated that you are personally responsible for some amount of the total bill. The statement you received lists the services you received and the amount you owe.

What If I Can't Pay Today?

If you believe there's a mistake on your bill, the amount you owe, or need more time to pay, we suggest you reach out directly to the treatment center listed on your billing statement to discuss your bill or a payment plan.
